Judicial Appeal Instructions

All students interested in filing an appeal for a determination made in the accountabily phase of a reported violation to the Student Code of Conduct should complete the form below. Please note you will need your case number when submitting this request. Your case number can be found on your decision/outcome letter that was delivered to you detailing the decision made in your case.

Any appeal of a finding or sanction must be completed within seven (7) calendar days of the receipt of the electronic notice of the original finding or sanction sent to your Hocking College email account

Grounds for Appeal: Check all that apply(Required)

1. Substantial Procedural Error: The existence of procedural errors so substantial that it greatly impacted the findings, responsibility determination, and/or the ultimate outcomes 2. Inequitable Finding and/or Sanctioning: The outcomes/sanctions imposed are substantially disproportionate to the severity of the violation. 3. Presence of New Information: Presentation of new and significant evidence which was not reasonably available at the time of the initial investigation, case resolution meeting, and/or hearing panel and would likely alter the findings, responsibility determination, and/or the ultimate outcome/sanctions.
